Job Description:

The Care Management Quality Consultant (CMQC) partners with system and medical group/ambulatory leaders to ensure care management quality assurance within the Ambulatory and clinic settings. This includes working with clinical audit plans and participating in continuous quality improvement projects to meet regulatory (including NCQA and the conditions of participation for Medicare), contractual, standards of practice, and documentation requirements for interprofessional care management teams. This consultant performs routine QA/clinical audits, data analysis, and identifies opportunities for improvement. Participates in the development of policy, procedures, guidelines, process flows, audit tools, and education plans to be organized and executed. Provides system care management education to ensure a high-quality program and continuous process improvement. Participates in regulatory and accreditation surveys. Assists in meeting and organizing surveyor requests to coordinate survey plans. Assists in orchestrating strong transitional care management plans and handovers to better coordinate care between CM teams.

Job Requirements:

Consults and mentors the CM IIs within Ambulatory CM concerning audits with feedback on results to ambulatory care managers.
Collaborate with Ambulatory Care Management leadership to design software solutions for reporting, validation of data, and requirements needed to meet regulatory standards as well as system goals and outcomes.
Support implementation of Ambulatory Care Management initiatives and policies utilizing project management, communication, and consultative skills.
Oversee continuous performance improvement/quality/safety quality projects in areas of responsibility as prioritized by Ambulatory Care Management Leadership.

Minimum Requirements:

Bachelor's Degree in Nursing from an accredited institution or a Master's Degree LCSW (degree will be verified).
Current state licensure.
Quality Improvement Training.
Two years of professional experience in a quality role that requires data gathering, analysis and education.
Two years of professional experience in Care Management.
Four years of patient care experience in discipline and demonstrated expertise in assigned clinical area(s).
Experience in a role requiring demonstrated competency in education principles and an understanding of adult learning principles.
Knowledge of various education/quality options to recommend the best solution to gain performance outcomes.
Experience in a leadership role requiring mentoring ability and an aptitude for coaching and consulting others.
Proficient in word processing, spreadsheet, database, internet and e-mail and scheduling applications.
Experience in a role requiring effective verbal, written and interpersonal communication skills.
Experience in a role requiring organization, communication and problem-solving skills and a demonstrated ability to prioritize competing priorities with an eye towards system-wide needs.

Preferred Qualifications:

Master's Degree Nursing, Continuous Improvement, or Quality related field.
National certification in area of expertise.
